This discussion also covers vaccination education for more than 200 participants through Zoom Cloud Meeting and UNS YouTube Channel. Starting the discussion, dr. Pompini Agustina S., Sp.P., (K)., a UNS alumnus from UNS Medical Science class of 1988, highlights the increasing trend on Covid-19 cases with medium, severe, and critical conditions. According to her experience, the conditions occurred due to the late treatment and delays in patients recognizing symptoms of the disease.<\/p>\n<p style=\"text-align: justify;\">On the other hand, she added, patients often have recognized the symptoms but have difficulty accessing health services because of the problematic referral system. \u201cFor instance, registering too many hospitals to get a referral, but in the end, it only added-up the long queues,\u201d dr. Pompini, who currently serves in RSPI Prof. Dr. Soelianti Sarosa Jakarta, states on Wednesday (20\/01\/2021).<\/p>\n<p style=\"text-align: justify;\">Responding to this condition, dr. Pompini mentioned the case management stages implemented at early detection, determination of disease severity, and appropriate handling according to severity. One of the main points she mentioned during her presentation is the early detection, which can assist the disease severity determination and the required handling.<\/p>\n<p style=\"text-align: justify;\">\u201cA weak early detection leads to weak disease severity determination. As consequence, the patients will be at late cases or severe cases when reaching health institution,\u201d dr. Pompini adds.<\/p>\n<p style=\"text-align: justify;\">Regarding the degree of severity, dr. Pompini states that minor Covid-19 cases with comorbidities require caution during the evaluation stage. Even though the severity is minor, but uncontrolled comorbidities (such as diabetes mellitus, heart disease, hypertension, chronic pulmonary) can turn into a severe illness. Meanwhile, for the medium severity, quick monitoring and treatment at a primary health facility are required. She hopes that the public will be more caring about the ongoing condition and do not wait for treatment. Even if it was just a cough or fever, they need to check with doctors to know whether they need hospitalization, oxygen assistance, antiviral drugs, or supportive medication. These steps also apply for severe cases, which does not rule out the possibility of entering a critical condition. At this stage, dr. Pompini stressed the importance of undelayed oxygen therapy treatment to avoid the critical condition.<\/p>\n<p style=\"text-align: justify;\">\u201cOn the other hand, a severe critical condition must be treated using mechanical breathing assistance. This is where the patients may get an additional infection such as bladder infection due to additional aids. This condition is the reason why Intensive Care Unit (ICU) handling is difficult. Therefore, (at this stage) we do not only consider the Covid-19,\u201d she explains.<\/p>\n<p style=\"text-align: justify;\">Additionally, dr. Arifin, Sp.PD., KIC., FINASIM., from Dr. Moewardi Hospital Surakarta shares his experience in the ICU. Due to the ever-increasing number of patients and unequal room availability, many patients with severe and critical conditions cannot always get treated in ICU.<\/p>\n<p style=\"text-align: justify;\">\u201cPatients with the severe and critical condition can be treated in ICU if available, if not, then they had to get treated at regular treatment room such as the ER,\u201d explains the alumnus of FK UNS class of 1992.<\/p>\n<p style=\"text-align: justify;\"><strong>Vaccination<\/strong><\/p>\n<p style=\"text-align: justify;\">On this occasion, also present the Director of UNS Hospital, dr. Tonang Dwi Ardyanto, Sp.PK.Ph.D., who discuss the Covid-19 vaccines. Specifically, dr. Tonang mentions the vast number of vaccine candidates currently under clinical test. Most of these vaccines like the one used in Indonesia was given by injection. Research on Covid-19 vaccines through nose inhale is limited. This case is related to the two antibodies that are formed when infection occurs, namely Immunoglobulin A (IgA), which takes place in the upper respiratory tract, and the Immunoglobulin G (IgG) in the lower respiratory tract. The injection was chosen because it will trigger strong IgG, which has a relatively long duration compared to IgA.<\/p>\n<p style=\"text-align: justify;\">Moreover, a person who was previously infected by Covid-19, when their IgG is strong the virus can no longer enter the lower respiratory tract and lungs because IgG is still present. \u201cIf the vaccines were given through the nose, the IgA will be strongly triggered but the IgG will be less triggered. In fact, IgA is quickly depleted. A person who previously infected by Covid-19 and got their IgA depleted can get re-infection in the upper respiratory tract,\u201d reveals the FK UNS alumnus class of 1993.<\/p>\n<p style=\"text-align: justify;\">Regarding the production of \u2018Merah Putih\u2019 Vaccines, dr. Tonang reveals that UNS took roles in establishing the road map. In other words, UNS is coordinating the developments from fellow universities and other institutions that are involved in this process.<\/p>\n<p style=\"text-align: justify;\">\u201cRight now, we cannot depend on \u2018Merah Putih\u2019 Vaccines. But hopefully, this step does not only apply for Covid-19 but also the future vaccines as well as encouraging the independence and sovereignty of Indonesian vaccines,\u201d dr.
